You throw away the block of wood and start again.

"If you run into a knot, you might throw away the whole piece," says carver Moe Thompson.

But this doesn't usually happen unless the flaw is large or simply can't be incorporated into the piece. It's a matter of judging the quality of the piece and reassessing the original design.

"You draw the figure to figure out what you want to do and how to make it pleasing to the eye," says Thompson. "You might even begin the figure first in clay or Plasticine to get an idea."

If you've invested this much time in a design you love, and the knot simply can't be worked around, you might be better off starting again. You need to think about many variables, including how far you are into the piece and time constraints.
