You are a nurse aide attending Rosaria, a frail elderly woman, at the extended care unit in the hospital. She is one of your more difficult patients because she is bitter and constantly complains. However, you don't let her foul moods get to you because you understand that she is just having difficulty coping with her frail condition.

You are attending another patient when you hear a crash, clatter and moan coming from her room. You rush to the bedroom and discover Rosaria lying on the floor.

"Help me! Help me! I'm dying. I'm paralyzed! Where were you? I'm in pain!" Rosaria moans and complains.

Is Rosaria just overreacting again? You check her for signs of bleeding, bruising and broken bones.

As you examine her, you notice bruising and a laceration on her lower leg. Your heart races. Has she injured or even broken her leg? Rosaria continues to scream that she is in extreme pain. But she is constantly complaining, so you don't really know how she feels.

"You constantly have to make decisions on what to do, and whether to call someone in for help," says Rhonda Crouch, a health-care assistant and nursing aide.
