Real-Life Decision Making

You have a 14-year-old patient, Lisa. You've diagnosed her as having a vision disorder that will lead to blindness. Her father, understandably, is upset with this diagnosis, and he is constantly interfering with your care of Lisa.

One day, he storms into your office and insists you give Lisa the same contacts you prescribed for his older daughter. You explain to this man that his eldest daughter's symptoms might seem like Lisa's but the disorder is quite different, so the contacts would be useless. Your explanations mean nothing to this man. He keeps insisting on the contacts.

You begin to wonder if you might be better off giving Lisa the contacts. After all, they can't hurt her at this point and might make her and her father feel more in control of a very distressing situation. However, you know these particular contact lenses are very expensive, and prescribing them would be like throwing this man's money away. It might also create false hope.

What would you do?
