You have guidelines to follow regarding transcript mailouts. The following information, in six steps, must be taken down:

Step One -- You must take down the name of the person requesting the transcript
Step Two -- You must get the person's student number or his former student number
Step Three -- You must take down his birth date, which aids in locating his records
Step Four -- You must find out when the person attended Zeke
Step Five -- You must get the person's mailing address
Step Six -- You must get the person's phone number, just in case you require further information
